Eligibility for Supervised Visitation 1. Must present most current Divorce Decree, ID cards, copy of Court Order (if applicable). 2. Identification must be presented at intake and thereafter.  3. All Intake documents must be completed prior to services.  ​4. Must sign-in and sign-out.  5. All clients are eligible, both private and CPS referrals
